Join Our Caring Team as a Care Co-Ordinator
Are you passionate about improving patient care and supporting individuals ? Join our friendly and supportive team as a Care Coordinator at Modbury Health Centre.
As a member of our team, you would bring integrity, kindness, and teamwork, as well as your skills to support our patients and team. We are an organisation of about 30 people working in Modbury. We provide a range of healthcare to our community totalling 5300 patients. We are lucky to have a team of 7 dedicated GPs working at the heart of our practice.
We form part of the South Hams Primary Care Network and work with Dartmouth/Chillington/Salcombe/ Kingsbridge, and you will also liaise and build relationships with colleagues in theses neighbouring surgeries and the wider community.
We are a small but supportive, friendly, and hardworking team who care about the people in the community we serve. We know the work can be challenging, supporting people when they may need it the most, so we endeavour to support and help each other. We all find serving the community and getting to know individuals, caring, and making a difference to their health and life is very rewarding
About the Role: As a Care Coordinator, you’ll play a key role in helping patients navigate health and care services. Working closely with GPs, clinical staff, and external partners, you will ensure patients receive timely, personalised, and coordinated care.
The Care Coordinator role is seen as a critical and evolving post to support the Enhanced Health in Multi-Disciplinary Teams within the locality to deliver effective, co-ordinated care for all patients including vulnerable and frail adults, particularly those at high risk of a hospital emergency admission, ED attendances or out of hours care.
Key Responsibilities:
- You will play a central role in shaping each patient’s experience from the moment they contact us either by telephone/face to face or online
- Coordinate care for patients with multiple health and social care needs
- Liaise with multidisciplinary teams (MDTs) to ensure joined-up care
- Help patients access services, appointments, and follow-up support
- Maintain accurate and up-to-date records
We’re Looking For:
- Clear, polite telephone manner
- Excellent organisational skills
- Excellent communication skills (written and oral)
- Good interpersonal skills
- Compassionate and patient-focused approach
- Ability to work independently and within a team
- Experience in primary care health or social care settings (preferred)
- Proficiency in IT systems and Microsoft Office
- Educated to GCSE level or equivalent -GCSE Mathematics & English (C or above)
Desirable:
- Knowledge of the NHS and local health care system
- Experience with SystmOne
- Care coordination or care navigation experience
